Who Can Benefit chronic disease management?
Individuals managing complex health conditions often require frequent hospital visits. BWL Social Care’s support will be there to support you through appointments, complex medication regimes, after appointment care for instance if you go in for treatment and find you come back needing support for a few hours or however long, or sometimes you or your loved one just needs someone to be with for emotional support, what is needed, you could count on us to be there to support, contact us.
Respite care services for carers:
Allow us to give you a break by looking after your loved one while you take time off to re energise , and have time to yourself to do things you enjoy or simply have a break. Caring responsibilities can be overwhelming, so allow us to step in.

We love to clean, so whatever you need from our carers, just let us know . We will do as much as you would like us to, and help you to clean and bring order to your house.
- Mental Health Support: We will help you to leave as full a life as you can by being there for you. We have a lot of experience in mental health so you can rest assured we will support with emotional support, creating safety plans with you, wellness action recovery plans support with coping mechanisms, and support you to access help when you need it.
We will support you with sorting out your mail and making those important phone calls, tenancy support, support you to go shopping or just socialise in the community, attend appointments, remind you to take your medication, and help you gain confidence in doing day to day tasks.
- Older Person Care: You or your loved one is guaranteed exceptional care, we look after our customers as though they were family and are protective of our older people ensuring first class personal care, good nutrition and hydration, a clean environment, medication support, pressure care support, catheter care, stoma care plus any other support you or they might need. Above all, we want them to feel loved and cared for.
- Personal Hygiene Care: If this overwhelms you or your loved one, we will take the journey with you and be there to support as much as we can. We will ensure you or your loved one is comfortable, feels respected and listened to as they walk the journey to becoming independent in this area. Been in hospital for far too long to manage on your own? Being discharged can be overwhelming
BWL Social care will help you relearn how to do daily activities, like cooking meals and washing. We can assist by supporting you or your loved one for a few weeks until you or your loved one is able to regain confidence to manage without support. Everything we do is person centred, so you will be in control of how long this lasts. Our aim is to ensure you achieve your goal to take charge of your life again.
